What are transition services?

The term “transition services” means a coordinated set of activities for a child with a disability that are designed to be within a results-oriented process. The services focus on improving the academic and functional achievement of the child with a disability to facilitate the child’s movement from school to post-school activities. These activities include postsecondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation.

Services are based on the individual child’s needs, taking into account the child’s strengths, preferences, and interests. This may include instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and when appropriate, acquisition of daily living skills and functional vocational evaluation. What do transition services look like at Goochland High School?

The Transition Specialist meets with students identified as having a disability throughout the school year either individually or in a classroom (group) setting. The focus of these meetings is centered on transition planning which incorporates progress towards mastering the transition goals stated in his/her current school year IEP. The transition exploration activities are tailored specifically based on the student’s strengths, preferences, interests, and needs. Transition services are results-oriented; therefore, each student’s transition pathway is geared to guide the student towards his/her post-secondary goals in relation to education, employment, independent living, and community participation.
